feat: add knowledge-loop skill and the global OKF knowledge bundle 9441a56353

Planning lessons and design patterns now have a home: one OKF v0.2 bundle per
project (openspec/knowledge/) and a global one in this repo (knowledge/). The
skill holds the whole procedure — read while planning, distill one lesson at
the end of doubt-review, run a retro before archive, and ask before promoting
a stable, repo-independent concept to the global bundle.

feat: read knowledge while planning and distill one lesson per doubt-review 30712951ba

Every quality/quality-ui artifact now applies matching lessons from the project
and global knowledge bundles, and design decisions record the Design Pattern
they adopt so the archive retro can confirm or revise it. The doubt-review
gains Phase 5: at most one Planning Lesson, backed only by fixed must-fix
findings, recorded in a new `## Lesson` section. check-plan requires that
section and verifies the lesson concept exists, is a Planning Lesson, and cites
this change's doubt-review.md.

Plans reviewed before this change need `## Lesson` + `NO_ENTRY` added to their
doubt-review.md; that file is outside the fingerprint, so no re-review.

feat: run the knowledge retro before archiving a quality change 11aa0325e1

OpenSpec 1.12 schemas have no archive hook; the archive workflow reads
operations.archive.guidance from the project's config.yaml instead.
enable-quality now adds a guidance entry pointing at the knowledge-loop
retro, which compares each D-n with the built code and confirms, rewrites,
or deprecates the patterns it used. Re-running it on an already-enabled
project adds the guidance; an existing operations: block is left alone and
the script fails with the line to add by hand.

chore: link the global knowledge bundle into ~/.pi/agent on bootstrap debea97bad

A symlink rather than a copy, so lessons and patterns promoted from any
project are written into this repo's knowledge/ and committed here instead of
drifting in a deployed copy. An existing real directory is left untouched with
a warning; the verification step checks the link resolves to an index.md.

docs: document the knowledge loop and its migration 0d16d14238

README covers the four steps (read, lesson, retro, promote), the new skill
and global bundle in the tree, and the one-time migration for plans reviewed
before the Lesson section existed. NOTES records the missing git identity on
this machine and that the archive retro is advisory only.
